Please note: Once on Riverchase Parkway, proceed approximately 1 mile
to the fourth traffic light and make a right onto Riverchase Office Drive.
Proceed up a small hill approximately 25 yards. The first left will be
marked 2100 River Chase Center. Do NOT turn there. Proceed a little
farther to your second left and make the turn. The street is not marked,
but there is an American Family Care sign on the right side of the road.
Proceed straight to the back of the office complex. The last building
is # 300, a one story brick building. The entrance to suite 316 is a
door on the outside of the building labeled "HCM of Alabama, Inc".

Directions from North of the Center (heading South):
Take I-65 South to exit # 247 Valleydale Road. Yield to the right off
the exit onto Valleydale Road. As soon as you merge onto Valleydale Road
get into the right hand turn lane. Proceed approximately 50 yards to the
first stop light. There will be a Shell gas station on the right. Make
a right onto River Chase Parkway (there is a large cement block in the
median of the road that says "Riverchase" on it).
Directions from South of the Center (heading North):
Take I-65 North to exit # 247 Valleydale Road. Merge to the right onto
Valleydale and proceed approximately 100 yards to the second stop light.
There will be a Shell gas station on the right. Make a right onto River
Chase Parkway (there is a large cement block in the median of the road
that says "Riverchase" on it).
Directions from East of the center (heading West):
Take 2059 West to I-65 South. Take I-65 South to exit # 247 Valleydale
Road. Yield to the right off the exit onto to Valleydale Road. As soon
as you merge onto Valleydale Road get into the right hand turn lane.
Proceed approximately 50 yards to the first stop light. There will be a
Shell gas statio on the right. Make a right onto River Chase Parkway

Directions from West of the center (heading East):
Take 2059 East to I-65 South. Take I-65 South to exit # 247 Valleydale
Road. Yield to the right off the exit onto Valleydale Road. As soon as
